- $50 Copay Labs / X-Rays
- Generic medications from $15
VitalCare covers everyday and preventive care in-network (MultiPlan PHCS): primary and specialist visits, urgent care, labs and x-rays, prescriptions, and preventive care. It does not cover emergency room, hospital stays, surgery, advanced imaging (CT/MRI/PET), maternity delivery, or mental health services. This plan provides Minimum Essential Coverage but does not meet Minimum Value Standards.
Community-Based Sharing
For large, unexpected bills — not insurance
- Hospital, Surgery, Accidents
- $1,250 Initial Unshareable Amount (IUA)
- 3 IUA maximum per household in any rolling 12-month period
- Eligible needs shared after your IUA
- See any provider for shared medical needs (no network on the sharing side)
